Position Overview
Purpose: The Nurse is responsible for providing care for the; traditional Medicare patient, specialized infusion therapies, wound care and professional nursing services to patients and caregivers at home or in alternate settings.
Job Responsibilities
Provides direct and indirect nursing care to patients based on a modified primary nursing approach. Such provision of care includes, but is not limited to: Patient assessment, clinical and environmental, Coordination of related services, Patient and caregiver teaching, Development and revision of the plan of care, Skilled nursing care, Implementation of physician orders, Monitoring of patient response, Response to emergency patient needs, Psychosocial support.
Completes all required patient care documentation and reports accurately and in a timely manner, according to internal and external regulatory requirements.
Communicates with service center personnel, physicians, and other health care providers to assure the quality and continuity of patient care.
Plans daily work activities in accordance with patient needs, assuming an on-call responsibility and extended patient related travel as required by the service area.
Takes professional responsibility for self-protection during required work in potentially hazardous situations (i.e. handling of cytotoxic agents, handling of blood and body fluids andEstimated Salary: $20 to $28 per hour based on qualifications.
